Jessica I did some more digging so I could provide you with references for the interpretation.

This is from the North American Consensus: A rise in hydrogen of ≥20 p.p.m. by 90 min during glucose or lactulose BT for SIBO was considered positive. Methane levels ≥10 p.p.m. was considered methane-positive. You can see they’ve not stated a rise for methane.

HOWEVER, Dr Siebecker wrote this article, where she states this criteria for positive methane: A rise over lowest preceding value in methane production of 12 ppm or greater within 120 minutes after ingesting lactulose. But she does not state a need for a rise in our course, just to be above 10ppm or 3-9ppm with constipation, so maybe this article is outdated (in fact, it must be because her criteria is now 10ppm and dates back to 2014). She does go on to say: We have found that an absolute level of gases at or above the positive ppm levels provided by QuinTron, without a rise over baseline or lowest preceding value, correlates well with clinical SIBO. This is especially true for methane gas, which can have a pattern of an elevated baseline (> 12 ppm) that remains elevated for the duration of the test (Figure 1). In cases such as these, methane may only rise 5 ppm, but the ppm level is consistently above the positive cutoff. Interpretation of elevated hydrogen or methane on the baseline specimen (pre-lactulose ingestion) is controversial, but we prefer to consider a high baseline value to be a positive test9, 10 (Figure 2) unless gas levels fall after baseline and continue to diminish or remain low during the first 2 hours, indicating improper prep diet (Figures 3 and 3.5).

So from all of that, my feeling is had your levels not continued to fall from the baseline, this would be a positive but they have fallen throughout the test, indicating perhaps a improper prep diet. I’ll be interested to hear what Jacobi thinks!

Julie - to be clear, if you have constipation then your levels of methane by 120 minutes would be a positive, if not, you do have methane but it’s not technically called methane SIBO because it’s in the large intestine (which is after 120 mins onwards) - it would be called Methanogen Overgrowth, but regardless, treatment is exactly the same. That’s why we look at the whole three hours, if it’s there, we treat!
